Chère lectrice, cher lecteur,

Séropositive et enceinte. Une situation difficile, si ce n’est dramatique pour la Sud-Africaine Phindile. Personne n’a pu la conseiller valablement, ni les puéricultrices, ni les infirmières, ni le médecin de l’hôpital. Sa mère a même récriminé contre elle, pensant que l’enfant d’une femme séropositive était lui aussi automatiquement séropositif. Dans de nombreux pays, les femmes enceintes séropositives subissent des pressions de la part du personnel de santé pour se faire avorter ou stériliser, rapporte la Communauté internationale de Femmes vivant avec le VIH (International Community of Women living with HIV). Mais Phindile ne se laisse pas décontenancer: «J’ai dis aux gens: oui, je suis séropositive, mais j’ai quand même le droit d’avoir un enfant.»

En Suisse, les femmes séropositives ont des possibilités de choix. Chez nous, il n’y a plus aucune naissance de bébé séropositif. Dans des conditions optimales, le taux de transmission de la mère à l’enfant peut être réduit à pratiquement zéro, cela grâce à la prise ciblée de médicaments contre le VIH durant la grossesse, à l’accouchement par césarienne, au renoncement à allaiter et au traitement préventif des nouveau-nés avec un médicament anti-VIH.

Malgré de gros progrès médicaux, seulement 9 pour cent des femmes enceintes séropositives dans les pays du Sud ont ces possibilités. Rares sont les hôpitaux qui proposent des programmes de réduction de la transmission mère-enfant du VIH (prevention of mother to child transmission, PMTCT) – programmes qui offrent aux futures mamans des informations et conseils, une surveillance prénatale, une prophylaxie médicamenteuse (pour la mère et l’enfant) et un suivi. La prise unique d’un seul médicament (névirapine) permettrait déjà de diminuer de moitié le taux d’infection. Ainsi, chaque année, 300’000 enfants seraient à l’abri du VIH, et leurs parents pourraient s’abandonner totalement à la joie de la naissance de leur bébé.

Le désir d’une Africaine d’avoir un enfant en bonne santé est-il donc moins légitime que celui d’une Suissesse? Quelles sont les possibilités et les limites des programmes de réduction de la transmission verticale du VIH? Quelle peut être notre contribution pour que le droit de toutes les femmes et de tous les hommes à une prise en charge globale face au VIH/sida (prévention, traitement, soins et soutien) devienne une réalité?

Nous aborderons et discuterons ces questions – et d’autres encore – liées à la thématique «positive motherhood» lors de la conférence 2007 d’aidsfocus.ch, le 26 avril à Berne. Bien entendu, vous êtes cordialement invité-e-s à y participer.

AIDSFOCUS.CH CONFERENCE: POSITIVE MOTHERHOOD - OPPORTUNITIES AND CHALLENGES OF HIV PREVENTION, TREATMENT AND CARE

Bern | aidsfocus.ch conference 26 April, 2007: If an HIV-positive woman becomes pregnant, there is a 35 per cent chance that she will transmit the virus to her child if no preventative action is taken. 95 per cent of pregnant women who are HIV-positive live in developing countries. More than 700,000 children become HIV-positive via vertical HIV transmission. However, this is preventable.

Every woman, every man must be guaranteed the right to decide whether and when to have children, irrespective of their HIV status. For many pregnant women, it is when seeking antenatal care that they learn about their HIV status. It is also the time and opportunity for HIV information, prevention, treatment and care of the women and their family members.

What are the challenges and opportunities of voluntary counselling and testing in antenatal care and of prevention of vertical HIV transmission? What are the criteria for breastfeeding by positive mothers and/or supplying infant formula? What is the impact of socio-cultural significance of motherhood on HIV prevention and care? These are some of the issues which will be explored at the conference, looking at them from a socio-cultural as well as a medical perspective, reflecting on theory and practice.

INFORMATIONS DE LA „COMMUNITY OF PRACTICE“ SUISSE


AIDSFOCUS.CH SOUTIENT LA PÉTITION DE MSF: „LES MALADES AVANT LES BREVETS“

Le groupe de pilotage d’aidsfocus.ch a décidé à l’unanimité d’apporter son soutien à la pétition lancée par Médecins sans Frontières dans le but d’exhorter Novartis à abandonner son action en justice contre le gouvernement indien et la loi indienne sur les brevets. Considérant que l’accès aux prestations de santé est un droit fondamental pour tout être humain, aidsfocus.ch plaide pour que toutes les personnes vivant avec le VIH puissent bénéficier d’un traitement antirétroviral gratuit ou, du moins, d’un prix abordable. Un durcissement de la loi indienne sur les brevets, comme le demande Novartis, entraînerait un renchérissement des génériques indiens avec lesquels une grande partie des pays en développement sont approvisionnés, ce qui aurait de graves conséquences pour les innombrables femmes, hommes et enfants qui vivent avec le VIH et qui dépendent de médicaments bon marché.

PRES DE 250 000 PERSONNES DEMANDENT À NOVARTIS DE RENONCER À SON ACTION EN JUSTICE EN INDE

New Delhi/Genève, 29 janvier 2007 – Alors que le procès intenté par l’entreprise pharmaceutique Novartis contre l’Etat indien reprenais devant la Haute Cour de Chennai, près de 250 000 personnes ont exprimé leurs préoccupations concernant l’impact négatif que l’action menée par le laboratoire pourrait avoir sur l’accès aux médicaments dans les pays en développement. L’association The Indian Network for People with HIV/AIDS (INP+), le People’s Health Movement, le Centre for Trade and Development (Centad), ainsi que Médecins Sans Frontières (MSF), ont de nouveau appelé Novartis à cesser immédiatement ses poursuites judiciaires en Inde.

NOVARTIS: EIN MUSTERKNABE IM REGEN

Das Basler Pharmaunternehmen Novartis versucht in Indien per Gerichtsverfahren, zu einem Patent auf sein erfolgreiches Krebsmedikament Glivec® (Imatinibmesylat) zu gelangen. PROFESSOR LÜTHY EUROPÄER DES JAHRES

Prof. Ruedi Lüthy, Gründer von Swiss Aids Care International, baute in Simbabwe eine Aidsklinik auf. Er erhielt am 18. Januar 2007 die Auszeichnung "Europäer des Jahres". Seit 1996 wählen die Chefredaktoren der 20 europäischen Ausgaben von Reader's Digest diese Persönlichkeit. Anlässlich der feierlichen Preisübergabe an der Universität Zürich hielt Frau Bundespräsidentin Micheline Calmy-Rey die Laudatio.

TERRE DES HOMMES SCHWEIZ AM WELTSOZIALFORUM 2007 IN NAIROBI

Unter dem Motto „Partizipative Jugendarbeit“ präsentierte terre des hommes schweiz am Weltsozialforum (WSF)in Nairobi Body Maps von 20 Jugendlichen aus aller Welt. Die Jugendlichen aus Kolumbien, Kenia, Moçambique, Südafrika, Tansania und der Schweiz trafen sich vor dem WSF in Nairobi, um in einem Workshop die Body Maps herzustellen.

ARRÊT D’UN ESSAI SUR LES MICROBICIDES

Dakar, 1 février - Un essai sur un microbicide destiné à prévenir l’infection au VIH chez les femmes a été interrompu prématurément, les premiers résultats ayant montré que le nombre d’infections chez les femmes recevant ce traitement était trop élevé. Cet essai, mené par Conrad, une organisation liée à l'agence américaine pour la coopération internationale USAID, en Afrique du Sud, au Bénin, en Ouganda et en Inde, avait pour but de déterminer le rôle d’un gel vaginal à base de sulfate de cellulose dans la prévention de la transmission du VIH chez la femme. WORLD SOCIAL FORUM: END TO HIV/AIDS. A TALL ORDER IN FACE OF VIOLENCE

Nairobi, 24 Jan - The issue of violence exacerbating the spread of HIV/AIDS, particularly in women, has remained a hot one at the World Social Forum (WSF). From Africa to Asia, activists are reiterating that violence against women remains a threat to the HIV/AIDS fight, and that without governments addressing the matter, winning the war against the disease will be an uphill task. TRADITIONAL HEALERS JOIN THE AIDS RESPONSE
  1. February - African traditional medicine is often the primary, and frequently the only, accessible health-care option for many people living in sub-Saharan Africa. Attempts to bring biomedical and traditional health care together for people living with HIV began in the early 1990s when the World Health Organisation recommended that traditional medicine be included in national responses to HIV.

AFRIQUE DU SUD: FONDER UNE FAMILLE, UN DROIT POUR TOUS

Johannesbourg, 4 janvier - De plus en plus de Sud-Africains séropositifs ont accès au traitement ARV qui prolonge l’espérance de vie. Par conséquent, les questions de la procréation et des moyens de fonder une famille en toute sécurité sont débattues. Les travailleurs sanitaires ne disposent d’aucun cadre officiel qui puisse les guider lorsqu’ils informent les malades séropositifs sur leurs droits et leurs choix en matière de reproduction. «Si j’ai décidé d’avoir un bébé, je sais ce qui m’attend, mais je sais aussi que je suis libre et que j’ai des droits», a conclu Phindi Malaza. «Je ne laisserai jamais quelqu’un me dire que je n’ai pas le droit de devenir mère.» (PLUSNEWS)

ECUADOR: PREGNANT WOMAN URGED TO BE TESTED AND TREATED FOR HIV

Guayaquil, Ecuador, 4 January 2007 – When Andrea, 20, discovered that her husband had HIV, she immediately got tested. Already five months pregnant, she discovered that she was HIV-positive. - About 8,000 cases of people living with HIV and AIDS have been registered in Ecuador, with women increasingly among those infected. In response, UNICEF-supported public service announcements (PSAs) on radio and television are now advising pregnant women in Ecuador to take a free, voluntary HIV test. (UNICEF)

SOUTH AMERICA: GETTING THE MESSAGE

January 2007 - The Pan American Health Organization has produced a DVD anthology of nearly 200 television spots about HIV that have been shown around the Americas region over the last ten years. The disc set “VIHdeo America” was compiled to share information and experiences and help encourage new approaches to AIDS communications and messaging.

WOMEN AND GIRLS LIVING WITH HIV/AIDS: OVERVIEW AND ANNOTATED BIBLIOGRAPHY

HIV/AIDS is both driven by and entrenches gender inequality, leaving women more vulnerable than men to its impact. This BRIDGE report - consisting of an overview, annotated bibliography, and contacts section - considers the specific challenges faced by women and girls who are living with HIV and AIDS. Women's social, economic, and legal disadvantage is exacerbated by a positive HIV status, and vice versa. Violations of women's social, economic, and legal rights in turn obstruct their ability to seek care, treatment and support, and to realise their sexual and reproductive health and rights (February 2007)

MAKING THE SEXUAL RIGHTS OF WOMEN LIVING WITH HIV COUNT

This issue of Exchange, produced in association with the International Community of Women Living with HIV/AIDS (ICW), focuses on the experience of women living with HIV. The issue focuses particularly on sexual and reproductive rights. It shows how the ABC (Abstain; Be Faithful; Use Condoms) approach, currently favoured by funding bodies such as USAID, ignores the complexity of human needs and desires. It also includes a report from India on women who have lost their land due to their HIV status, and a report from Uganda on the ’memory work’ project in which mothers living with HIV and their children have been involved. LET'S TALK ABOUT HIV COUNSELLING AND TESTING - FACILITATORS' GUIDE

This toolkit is designed to help NGOs, CBOs and other civil society organisations responding to HIV/AIDS in developing countries increase their knowledge and improve the quality of their work on HIV counselling and testing. The toolkit has eight sections covering different aspects of HIV counselling and testing. Each section begins by providing essential information comprising key definitions, concepts and messages after which participatory activities are presented for carrying out with NGO/CBO staff. CHILDREN AND AIDS: A STOCKTAKING REPORT

‘Unite for Children, Unite against AIDS’ was launched in October 2005 with the goal of putting the ‘missing face’ of children at the centre of the global HIV/AIDS agenda. This new Report by UNICEF shows that in several countries in Eastern and Southern Africa, trends in access to antiretroviral regimens for preventing mother-to-child transmission are starting to show remarkable gains. Increasing numbers of children living with HIV are now receiving treatment, although the numbers are far too few. The increases are a result of improved testing, better skills among health workers, lower drug prices and simpler formulations. There are, however, huge gaps in progress. (UNICEF 2007)

LEARNING FROM EXPERIENCE: BEST PRACTICE COLLECTION

There are literally thousands of AIDS-related programmes around the world. To help drive the knowledge-sharing process, in 1997 UNAIDS began the production of its ‘Best Practice Collection’ — a series of publications ranging from guidelines, updates and policy papers to case studies, handbooks, and examinations of particular challenges and responses, all designed to promote learning, share experience and empower people and partners engaged in the AIDS response. The Best Practice Collection provides a wealth of ‘hands on’ experience that is useful and may be replicated. (UNAIDS 2007)

ASIA: FACING THE HIV/AIDS CHALLENGE

PlusNews Special Report documents that South East Asia has the largest number of people living with HIV after Africa, and its share of the global epidemic is growing - an estimated 8.6 million people in the region were living with the virus in 2006, with the epidemic expanding faster than anywhere else in the world. Because of the size of the region's population, relatively modest HIV prevalence can translate into millions of cases. Unless determined action is taken now, the virus could move into the general population. ( 2007)

Winterthur | Linus G. Jauslin, Mitgründer der Stiftung Aids & Kind, spricht im Rahmen der Ausstellung „ohne Dings kein Bums“ vom 1. März bis 4. April 2007 von der der Not von afrikanischen Kindern und Jugendlichen, die von AIDS betroffen sind. Seine persönlichen Projektberichte aus Kamerun, Sambia, Simbabwe und Südafrika machen das Elend der betroffenen Kinder greifbar und zeigen die Hilfsbedürftigkeit der Schwächsten unserer Welt. Er vermittelt aber über die Hilfeleistungen und Projektarbeit von Aids & Kind auch Mut und Zuversicht.

Winterthur | Im Rahmen der Veranstaltung “ohne Dings kein Bums” bietet Irene Bush, terre des hommes schweiz einen aktiven Workshop zu Memory Work und Hero Books an. In ihrem Workshop für Lehrpersonen wird die Methode von Memory Work erklärt und ein Hero Book, ein Heldenbuch, erarbeitet, eine Strategie für alle Jugendliche mit Schwierigkeiten umzugehen. «Auch Helden haben Probleme, aber sie gehen sie mutig an» meint Irene Bush, Vielleicht möchten Sie als Lehrer/in praktisch erfahren wir Memory Work funktioniert und, wie die eigene Biografie eine Quelle der Kraft wird?

Ausserholligen | The course offered by SDC aims to provide information on and skills in mainstreaming HIV/AIDS. It aims to strengthen participants’ motivation and competence to mainstream HIV/AIDS as relevant for their work. At the end of the day, participants should understand the concept of “mainstreaming HIV/AIDS” and its multisectoral dimension and know the most important elements of the SDC toolkit “mainstreaming HIV/AIDS in practice” and understand how to work with the toolkit.

Bern | Sharing of experiences and learning from each other is an effective way in the process to mainstream HIV into one’s own organisation. The Swiss Red Cross is inviting partners of aidsfocus.ch to the next meeting. The focus will be on sharing of experiences and information on monitoring and evaluation of mainstreaming HIV and AIDS. Participants preferably have participated in the SDC training module or studied one of the toolkits in order to make sure that we share a common basic understanding of the concepts. No formal group will be created - so attendance to the meetings can be decided case by case.
