«Le traitement antirétroviral pour tous, est-ce que cela englobe les enfants?», m’a demandé Stefan Germann, de World Vision International, à la suite de mon invitation au forum d’aidsfocus «Traitement antirétroviral pour tous – un projet réaliste» du 30 septembre 2008.

Oui, bien entendu, lui ai-je spontanément répondu, en pensant aux demandes répétées de Médecins Sans Frontières (MSF) pour la mise au point de médicaments antirétroviraux spéciaux pour les enfants – MSF qui a traité de par le monde plus de 10'000 enfants infectés par le VIH.

Au sein d’aidsfocus.ch, les enfants ont d’ailleurs un assez puissant lobby. Bon nombre des organisations participant à la plate-forme placent le bien-être et la santé des enfants au cœur de leur travail. Au début, l’attention était centrée sur les enfants qui avaient perdu leurs parents à cause du sida – les orphelins du sida. Mais la réalité d’aujourd’hui oblige nos partenaires du Sud à se tourner davantage vers les enfants eux-mêmes séropositifs pour répondre à leurs besoins. Deux exemples: Swiss AIDS Care, au Zimbabwe, cherche elle-même à combiner des médicaments pour le traitement d’enfants séropositifs, et l’association «Le sida & L’Enfant» soutient en Afrique du Sud le projet «My living positively handbook» qui s’adresse aux enfants vivant avec le VIH.

A l’heure actuelle, selon les données les plus récentes, plus de 2 millions d’enfants vivent avec le VIH/sida dans le monde. La plupart d’entre eux ont été infectés à leur naissance. Sans traitement antirétroviral prolongeant leur espérance de vie, la moitié des enfants séropositifs décèdent avant l’âge de deux ans. L’an dernier, 270'000 enfants sont morts du sida. On pourrait pourtant les sauver.

«Le traitement antirétroviral pour tous, est-ce que cela englobe les enfants?» est une question tout à fait justifiée et qui n’est rhétorique qu’au premier abord. Dans les discussions et stratégies internationales de lutte contre le VIH/sida, les enfants sont largement négligés, ont constaté les spécialistes réunis début août à Mexico à l’occasion de la Conférence internationale sur le sida. Selon ces experts, les enfants ont plus difficilement accès que les adultes aux médicaments antirétroviraux; les données relatives à la jeune population sont très insuffisantes et, en même temps, le savoir disponible n’est souvent pas pris en considération. Ce qui fait défaut, ce sont des méthodes diagnostiques peu coûteuses et des médicaments sur mesure pour les enfants – les enfants d’Afrique ne constituent pas un marché pour les entreprises pharmaceutiques.

Bern | aidsfocus.ch Forum on successes and challenges of HIV treatment in resource-limited settings.

Successful therapies have shown that if existing resources are invested in a good and wise manner a lot can be achieved in resource-poor countries of the South as well. The evaluation of the achievements of the integrated HIV and AIDS treatment and care programme of the Swiss Red Cross in Swaziland is the starting point for discussion. Médecins Sans Frontières (MSF) has demonstrated successful HIV therapies in 32 countries in Africa, Asia and Latin America. A recent study by the Berne University even comes to the conclusion that ARV treatment in slums of South Africa is as effective as it is in Switzerland.

The forum organised by aidsfocus.ch in cooperation with the Swiss Red Cross takes up these burning issues and based on concrete evidence opens up the discussion on challenges, approaches and strategies regarding comprehensive HIV prevention, treatment and care.

Mexico City, 3 August 2008 - “Children have been short-changed in the response to AIDS. They are visible in the photo opportunities and headlines, but mostly invisible in the response to HIV," Professor Linda Richter, of the Human Sciences Research Council of South Africa, told the XVIIth International AIDS Conference here today in a plenary address entitled "No Small Issue: Children and Families". Richter's is the first plenary address to be devoted to the wellbeing of children affected by HIV and AIDS in the conference's 23-year history. While the global response to AIDS has accelerated, prevention, treatment and care, children continue to lag behind. (HSRC)

Mexico City, 2 August 2008 - MSF speakers shared their experience at the Teresa Group Symposium "Children & HIV/AIDS: Action Now, Action How", discussing challenges faced in the field in delivering pediatric ART, reflecting on how these challenges are being addressed—or not—and what needs to be done to optimize care for children with HIV/AIDS in resource-limited contexts. Friedrichsdorf / Mexico City, 8. August.2008 - Stärker als bei früheren Welt-Aids-Konferenzen wurden in Mexiko auch die Auswirkungen der Epidemie auf Kinder thematisiert, stellten World Vision-Experten fest. Sie berichteten von Erfahrungen aus der Praxis, die dass in Afrika regelmäßige Hausbesuche durch geschulte Nachbarschaftsnetzwerke und Gruppentherapien den psychischen Zustand der Kinder und ihrer Betreuer sehr gut stabilisieren können, … eine wichtige Voraussetzung dafür, dass die nächste Generation ein eigenes Leben aufbauen kann – trotz Aids.“

The ‘Children’s ART Literacy Toolkit for Communities’ developed by SAfAIDS in Zimbabwe is using the engaging method of “edutainment” for children – fun learning! – and it has been designed for all children and those in their “circle of care” to provide child-friendly messages and activities to understand HIV and AIDS, strategies for managing treatment in their daily lives, hope and a platform for achieving their dreams.(August 2008)

Researchers of the Insitut of Social and Preventive Medicine (ISPM), University of Bern, in cooperation with the University of Cape Town, South Africa, compared the data of more than 2000 HIV-positive patients in South Africa with the data of over 1000 Swiss patients. Conclusions: “Compared to the highly individualised approach in Switzerland, programmatic HAART in South Africa resulted in similar virologic outcomes, with relatively few changes to initial regimens.”(PLoS Medicine, July 2008) The study will be presented and discussed at the aidsfocus.ch forum 30 September 2008 in Bern

Berne, 8 août 2008 - Les chiffres de l’ONUSIDA « témoignent de l’importance toujours actuelle de l’engagement à la fois international et multisectoriel dans la lutte contre le sida. Et expliquent pourquoi le VIH/sida reste un thème prioritaire pour la DDC…. Tous les secteurs de la DDC (coopération bilatérale et multilatérale, aide humanitaire) sont engagés dans ce combat, lequel devrait encore s’intensifier à l’avenir…. Mexico City, le 8 août 2008 - L’Aide Suisse contre le Sida considère que sa participation à la Conférence internationale sur le sida a d’une part permis d’avoir un accès direct à un volume considérable d’informations, tout en lui offrant d’autre part l’occasion de rencontres informelles avec des représentants des cercles médicaux, politiques, des personnes touchées et d’autres organisations du domaine. En octobre, l’Aide Suisse contre le Sida consacrera un colloque national au transfert des informations réunies et de leur analyse approfondie, aux impulsions acquises ainsi qu’à une discussion sur les tendances et leurs conséquences pour le travail contre le sida en Suisse.

Juni 2008 - Gewalt in Zimbabwe: SolidarMed musste seine Mitarbeitenden als letzte Schweizer Hilfsorganisation zurückziehen. Der temporäre Rückzug war notwendig geworden, nachdem die Unruhen und Gewaltaktionen bezüglich der bevorstehenden Präsidentschaftswahlen zugenommen hatten. Mexico/Genève, le 3 août 2008 – Médecins Sans Frontières (MSF) a lancé un cri d'alarme à propos de l'impact mortel du manque de personnel de santé sur le traitement du sida en Afrique australe. Lors d'une réunion satellite intitulée "Mind the Gaps" ("Alerte aux manque") organisée par MSF, des experts ont décrit l'ampleur et les conséquences de cette crise ainsi que la nécessité criante pour les gouvernements et les bailleurs de fonds de redoubler d'effort et de prendre des mesures concrètes et immédiates afin de conserver et de soutenir le personnel médical.

Mexico City, 7 August 2008 - Speakers at the XVII International AIDS Conference (AIDS 2008) underscored the importance of simultaneously scaling up AIDS programmes and strengthening health systems in poor countries, emphasizing that the two goals should be viewed as allies, not adversaries. Experts warned that the global shortage of health care workers hampers both goals and demands a collective response. (AIDS 2008, official press release)

Mexico City, 7 August 2008 - The recent controversial statement by Swiss researchers that HIV-positive people on treatment with an undetectable viral load could not infect their sexual partners was a hotly debated issue at the International AIDS Conference this week. Pietro Vernazza, president of the Swiss Federal Commission for HIV/AIDS (CFS) and the main author of the paper, said the findings gave hope, especially to discordant couples wanting to conceive. But Catherine Hankins, chief scientific adviser at UNAIDS, warned that the Swiss statement had "a large potential of doing more harm than good" - particularly in developing countries. (PlusNews)

Mexico, 8 août 2008 - La contribution des organisations confessionnelles au traitement et à la prise en charge des personnes vivant avec le VIH est bien connue, mais savoir si la religion est une aide ou une barrière lorsqu’il s’agit des efforts de prévention du VIH est moins évident. L’Eglise jouait un rôle majeur dans la prise en charge des orphelins et pour encourager les gens à aller se faire dépister. Mais la réponse de la communauté religieuse au VIH et au sida n’a pas toujours été positive. Des leaders et des groupes religieux ont contribué à la stigmatisation des personnes vivant avec le virus et à des attitudes négatives envers les minorités sexuelles. (PlusNews)

1. August 2008 - The journal Science examined the "intense scrutiny" that HIV/AIDS prevention and treatment received during last week's XVII International AIDS Conference in Mexico City. Mike Cohen of the University of North Carolina-Chapel Hill at the conference said the two efforts "keep going to the altar," but "[t]hey never get married. They have to get married today." Science reports that three million people in low- and middle-income countries now have access to antiretroviral drugs but that an estimated five people contract HIV for every two provided with treatment.(kaisernetwork)

1. September 2008 - The Third High-Level Forum on Aid Effectiveness takes place this week in Ghana. To mark this event, UNAIDS begins a new web series “Making the money work”. In coming months it will explore different aspects of aid effectiveness; how financial aid to countries can be made more effective, accountable and results-oriented. The Third High-Level Forum on Aid Effectiveness (HLF) takes place 2-4 September 2008 in Accra and will bring together ministers from over 100 countries along with donors, civil society, UNAIDS, its Cosponsors and other development agencies. Ce rapport examine les progrès réalisés au cours des quelque douze derniers mois dans quatre domaines où le VIH et le SIDA affectent les enfants. Le rapport fait valoir que la plupart des pays ont obtenu des gains importants dans la prévention de la transmission du VIH de la mère à l'enfant et dans le traitement pédiatrique. Certains pays ont progressé vers le but en matière de prévention du VIH et on compte davantage d'enfants qui bénéficient de services de protection, de soins et d'un soutien. Mais il reste beaucoup plus de choses à faire. Le rapport explique qu'il est nécessaire d'améliorer les normes, standards et directives pour assurer une mise en ouvre efficace des programmes. There is increasing evidence that introducing antiretroviral treatment (ART) in resource poor settings can be very effective. ART decreases AIDS morbidity and mortality, while also reducing the probability of transmission by lowering the patient’s viral load. In Africa, Asia, and Latin America, where 90 percent of people with HIV/AIDS live, access to ART has been limited due to the high cost of drugs and lack of infrastructure capable of large-scale ART. En plus de participer à son financement, elle y envoie des pédiatres et infirmières suisses. Quels sont les défis qu’implique la collaboration avec une grande organisation américaine? Que faire quand il y a discordance d’idées et de conceptions entre cette organisation, le personnel haïtien et nous-mêmes? Les participant-e-s à ce «meeting point» soulèveront des questions et en discuteront, faisant valoir leurs propres expériences et se conseillant mutuellement.
